BREAST SCREENING INFORMATION - SEE 'NEWS' SECTION

CHRONIC DISEASE CLINICS -  The surgery is changing recalls for patients to ONE recall in your birthday month for ALL chronic disease reviews:- diabetes, heart disease, asthma, COPD and stroke/TIA.  This is to reduce the amount of reviews you are asked to attend.  If you have more than one of the above conditions you will only need to be seen once at the surgery.

New Patient Registration

If you live within our practice area (Cleethorpes/Humberston) you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so it would help if you could provide proof by way of a recent utility bill.

Criteria for acceptances are:

  • New births to existing patients.
  • Immediate family members ie husband/wife/partner/child at the same address
  • Close family members ie parents, providing they reside at the same address
  • Patients moving in from outside the area

 

If you apply to register on-line please allow 7 days  before you contact the surgery to enquire if you have been accepted. If you are accepted you will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration. Please contact reception for further information.
